What is Barbie Botox?

Barbie Botox is botulinum toxin injected into the upper trapezius, the muscle that runs from the base of the skull across the top of the shoulder. Relaxing that muscle lets the shoulder line sit lower and flatter, which makes the neck read longer and the jawline read more defined. The names Traptox, trapezius Botox and shoulder slimming all describe the same treatment.

It is worth being precise about what this is. Injecting the trapezius with botulinum toxin is long established in medicine for cervical dystonia and for myofascial neck and shoulder pain. Using the same injection purely to reshape the shoulder line is a cosmetic, off label application. Off label is not the same as unstudied or improper, but you deserve to know which category your treatment sits in before you book.

What it does not do

It does not remove fat. It does not remove tissue. It does not lengthen your neck, because bone does not move. It relaxes a muscle, and everything you see in a before and after photo follows from that one change. If the bulk across your shoulders is fat rather than muscle, toxin will do very little and you should be told so at consultation rather than sold a treatment.

From our injection team: the honest test is whether the bulk hardens when you shrug against resistance. If it does, it is muscle and toxin has something to work with. If it stays soft, it is not, and we will tell you that in the room.

How the treatment is done

A small number of injection points per side into the upper trapezius, placed by palpating the muscle belly while you shrug. It takes about fifteen minutes. There is no downtime, you can drive yourself home and return to desk work immediately. Pinpoint bruising is possible.

We ask you to skip heavy pressing and heavy shrugging for the first 48 hours, not because the toxin migrates in any dramatic way, but because we would rather it settle exactly where it was placed.

Dosing, and what the evidence actually shows

Published work on trapezius contouring uses roughly 20 to 50 units per side. A 2025 systematic review by Kapoor and colleagues pooled the available studies and reported the contouring effect lasting about four to five months, which makes this a repeating treatment rather than a one time change. Budget for it that way.

That same review is also the reason we talk about this treatment carefully. It pooled nine studies and 121 cases in total. That is a small evidence base for something being marketed as routine, and anyone telling you it is completely proven is overselling it. Within those cases, about eleven percent had mild trapezius weakness that took one to three months to resolve, with no long term complications reported.

Our dosing sits in the conservative half of the published range on a first treatment. You can always add at the two week review. You cannot remove toxin once it is placed.

What to expect, week by week

Days 1 to 3

Nothing visible. Some patients report a mild heavy or tired feeling across the shoulders. Pinpoint bruising, if it happens, settles in a few days.

Week 2

Onset. The muscle starts to soften and many patients notice tension headaches and shoulder tightness easing before they notice any change in shape. This is the review point where we assess whether more units are warranted.

Week 4 to 6

Visible change. The slope from neck to shoulder softens and the shoulder line drops slightly. This is when other people start to comment.

Month 4 to 5

The contouring effect fades and movement returns fully. Most patients who continue rebook around this point.

Risks, and who should not have this

The trapezius is not a decorative muscle. It stabilises and rotates the shoulder blade and it is working every time you lift overhead, carry a bag or train. That is exactly why dose and placement carry more consequence here than they do in a forehead, and why the weakness figure in the literature is worth taking seriously.

We will decline or defer this treatment if you are a competitive or heavy overhead athlete in season, if you are pregnant or breastfeeding, if you have a neuromuscular disorder such as myasthenia gravis, or if your shoulder bulk is not muscular on examination.

Why the injector matters more here

Forehead toxin is forgiving. The trapezius is a large, deep, functional muscle sitting near structures you do not want relaxed, and the cosmetic evidence base is still thin. Three questions worth asking any clinic before you book anywhere, including here.

  • Who is physically performing the injection, and what is their licence?
  • What dose per side are you using on me, and why that number?
  • What is the plan if I end up weak?

If a clinic cannot answer the third question, that is your answer.

Frequently asked questions

Does it hurt?

Less than most people expect. The trapezius is a large muscle and the needles are fine. Most patients describe pressure rather than sharpness.

Will I get weaker at the gym?

Some patients notice reduced shrug strength for the first weeks. In the pooled literature about eleven percent had mild weakness that resolved within one to three months. If you train seriously, tell your injector before dosing, not after.
