What a lip flip actually does

A lip flip is a small dose of neuromodulator (Botox, Dysport, Nuceiva) injected into the orbicularis oris muscle along the upper lip. Typical dose is 4 to 6 units of Botox or equivalent. The neuromodulator partially relaxes the orbicularis oris, allowing the upper lip to roll outward slightly when at rest. This makes more of the vermillion (the red-pink part of the lip) visible, creating the visual impression of a slightly fuller upper lip without adding any volume. There is no gel, no syringe of filler, no immediate volume change. The mechanism is purely muscle relaxation.

What lip filler actually does

Lip filler is hyaluronic acid gel (Stylage, Stylage XC, Revanesse Lips Plus, RHA, Teosyal) injected into the lip body, vermillion border, philtral columns, or cupid bow to add volume, define shape, correct asymmetry, or address vermillion border definition. The result is immediate and physically present: there is gel in the lip taking up space and shaping the lip outline. Common volumes are 0.5 mL (subtle) to 1.5 mL (more visible) per session.

Three notable changes. First, lip flip dosing has been refined; lower doses (2 to 4 units total) are now common at top-tier clinics versus the 6 to 8 units that were standard earlier. Lower doses give the desired flip effect with less risk of functional impact on whistling, drinking through a straw, or pronunciation of certain consonants. Second, newer HA fillers like Stylage with IPN-Like cross-linking and Stylage XC reformulated for smoother flow have changed the landscape of subtle-result lip filler. Third, microcannula technique for the lip body has become standard, reducing bruising and overall procedure risk.

Patients with gummy smiles (excessive maxillary gingival display when smiling) gain substantial benefit because the relaxation of the upper lip elevators reduces the upward pull during smiling. Why some patients get better lip filler results than others

Lip filler works best when there is a clear deficit to address (asymmetry, volume loss, undefined vermillion border, lost cupid bow) and the patient has a clear realistic expectation. Patients who want lips that are not anatomically possible for their face structure (very large lips on a small chin and jaw, for example) will be unsatisfied even with technically excellent injection. What changes when you stop maintaining lip flip or lip filler

Patients sometimes worry that stopping lip flip or lip filler will leave their lips worse than before they started. This is a common myth. When lip flip wears off (6 to 10 weeks), the upper lip simply returns to its baseline position. The muscle is not weakened or atrophied; the temporary effect dissipates. When lip filler metabolizes (9 to 14 months), the hyaluronic acid is gradually broken down by the body, the gel volume reduces over months, and the lip returns to its starting shape. Filler does not stretch or permanently alter the lip; there is no lipstick lip deformation if you stop. The only meaningful exception is patients who have repeated over-filling for years, which can theoretically affect tissue elasticity.
