The jawline softens, the corners of the lower face round off, and a small step forms in front of the chin. The sharp border you used to have blurs into jowls. It is one of the clearest signs of lower-face ageing, and it is also one of the most treatable without surgery when it is caught early.

What is a sagging jawline, and what are jowls?

A sagging jawline is not one thing. It is usually a stack of changes that happen together:

  1. Bone resorption. The jaw and chin lose a little bone over time, so the framework holding the soft tissue shrinks.
  2. Fat descent and loss. Mid-face fat pads slip downward and the deep support thins, so volume collects along the jaw as jowls.
  3. Skin laxity. Collagen and elastin decline, so the skin envelope no longer snaps back.
  4. Platysma pull. The flat neck muscle pulls down on the lower face, which deepens the jowl and softens the line.

The jowl itself is the pouch that forms in front of where a ligament tethers the skin to the jaw, with a dip just ahead of the chin called the pre-jowl sulcus. Knowing which of these is driving your result decides which treatment actually helps.

Why does the jawline sag over time?

  • Age. Collagen, bone, and fat support all decline gradually.
  • Genetics. Lower-face ageing patterns run in families.
  • Sun damage. UV degrades collagen and elastin in the skin envelope.
  • Weight changes. Repeated loss and regain stretch the skin.
  • Hormonal change. The drop in oestrogen around menopause speeds collagen loss.
  • Smoking. Accelerates skin ageing and breakdown of support.

What treatments do we offer for a sagging jawline in Toronto?

This is treated with a mix of injectable and non-injectable options. The injectables rebuild structure and definition, and the energy devices tighten the skin envelope. The right plan layers them rather than relying on one.

Jawline and pre-jowl filler, structural definition

The single most effective non-surgical move for moderate jowls is restoring the jaw border with a firm filler. Placed along the angle and body of the jaw and into the pre-jowl sulcus, it straightens the lower-face contour, sharpens the line, and reduces the visible step. It will not erase a true jowl, but the lower face reads as more defined.

Cheek filler for support from above

Restoring volume in the cheek lifts the tissue that has descended and softens the jowl by association. Treating the cheek first is often the smarter sequence.

Botox to the platysma, the Nefertiti lift

When the platysma is actively pulling the jawline down, Botox along those bands relaxes the downward tension and gives a subtle lift to the jaw border. Botox is $10 per unit, and we also offer a dedicated Nefertiti Lift.

For a heavily lax skin envelope, deep jowls, and prominent neck banding, no non-surgical treatment matches a properly done face or neck lift. We do not over-fill surgical candidates. When surgery is the right answer, we say so and refer.

Injectable versus non-injectable for the jawline

Filler, Sculptra, Radiesse and platysma Botox are injectable, and they rebuild structure, replace lost support, and relax the muscle pulling the jaw down. Morpheus8, Forma and PDO threads are non-injectable (threads are placed but add no volume), and they tighten or reposition the skin envelope. A sagging jawline almost always responds best to a layered plan rather than a single treatment, because structure and skin quality are separate problems.

What combination approach makes sense?

For someone in their mid-40s to mid-50s with moderate jowls and early laxity, a typical plan is:

  1. Cheek support with filler or Sculptra to lift from above.
  2. Jawline and pre-jowl definition with filler along the border.
  3. Platysma Botox to ease the downward pull, every three to four months.
  4. A Morpheus8 course for skin tightening.
  5. At home, a prescribed retinoid and daily mineral SPF.

What results can I expect, and how soon?

  • Day 0: immediate jawline definition from filler.
  • Weeks 1 to 4: filler settles to its final look.
  • Week 6: early tightening from Morpheus8.
  • Months 3 to 6: the compounded result from Sculptra and Morpheus8 comes in.

What you can expect: a noticeably sharper jaw border, a softer pre-jowl step, a subtle lift from platysma Botox, and a firmer skin envelope from energy work. Better photos from the side and three-quarter angles.

What you cannot expect: a surgical facelift result, full correction of heavy skin laxity, permanent results, or a 30-year-old jawline at 55.

Who is a good candidate, and who is not?

The best non-surgical candidates have early to moderate jowls with a reasonably good skin envelope. Treatment should wait or change in these cases:

  • Pregnancy or breastfeeding.
  • Skin laxity that is honestly surgical, where we refer.
  • Blood thinning that cannot be paused.
  • Active infection in the area.
  • A pacemaker, specifically for Morpheus8.
  • Body dysmorphic concerns, which our MD screens for.

Intake and photos from several angles, including profile and three-quarter. A skin pinch test for envelope laxity. Palpation of the jaw structure. Identification of any platysmal bands. Common questions

Will filler give me a facelift?

No. Filler restores structure and softens the jowl. A facelift physically removes excess skin and repositions deeper tissue. They are different tools for different stages.

Will I look puffy?

Not with a conservative, layered approach. The goal is a defined jaw, not an over-volumised lower face.

How many syringes will I need?

It varies with your anatomy. Many jawline plans combine the jaw, chin, pre-jowl, and cheek, and we map the exact amount at your consult.

Does it hurt?

With a cannula and numbing it is usually around 3 to 4 out of 10.

How long do the results last?

HA filler is roughly 12 to 18 months, Sculptra well over a year, Radiesse around 9 to 12 months, Morpheus8 12 to 18 months, and platysma Botox 3 to 4 months.

Will I bruise?

Some bruising is common with jawline filler and usually settles within a couple of weeks. The pre-treatment instructions we send reduce it.

Can I do this before a wedding or event?

Yes, with timing. Allow about six weeks for filler, three weeks for Botox, and six to eight weeks for a Morpheus8 result.

Do threads actually work?

For moderate descent, yes, temporarily, with a lift that lasts in the range of a year to 18 months.

Should I just get a facelift?

For significant laxity, often yes, and we will refer you. For early to moderate change, non-surgical work does very well.

What about Ultherapy or Sofwave?

Those are focused ultrasound tightening devices. We do not offer them, and prefer Morpheus8 for its cost and result profile.

Can men get jawline treatment?

Yes, and many of our patients are men. We use a more angular placement to keep the jaw masculine.

What if I am losing weight on Ozempic or another GLP-1?

We wait for your weight to be stable before treating, because significant loss redistributes facial fat and you would otherwise redo the work.

Can I dissolve HA filler if I do not like it?

Yes. Dissolving filler is $260.

How do I know if I am a surgical candidate?

We will tell you honestly at the consult. Heavy laxity, deep jowls, and prominent neck banding usually need surgery for the best result.
