Two lines run from the corners of your mouth down toward your jaw, and your resting face reads as sad or stern when you feel neither. Those are marionette lines, and they are one of the most common reasons people in their 40s and 50s come in feeling like their expression no longer matches their mood.

What are marionette lines, really?

They are the static creases that run vertically below the corner of the mouth down toward the jawline. Three things meet here: the depressor anguli oris muscle, often shortened to DAO, which pulls the mouth corner down; the fold of tissue beside it; and the dip just in front of the chin called the pre-jowl sulcus. A small ligament tethers the skin along the line while the soft tissue on either side shifts with age. The line itself is the boundary, not the cause, which is the key to treating it well.

Why do marionette lines get worse over time?

It is a cascade rather than a single cause:

  • Mid-face volume loss. Cheek fat pads thin and slide down from the mid-thirties, pooling above the fold and deepening the line by contrast.
  • Bone resorption. The upper and lower jaw lose a little structure with age, so there is less support and more descent.
  • An overactive DAO. A strong depressor muscle pulls the corner down with every expression.
  • Lip volume loss. Thinner lips turn the corners down further.
  • Chin muscle activity. A busy mentalis deepens the pre-jowl dip.
  • Lifestyle. Smoking, sun, and side-sleeping all wear down collagen.
  • Rapid weight loss. GLP-1 medications like Ozempic and Wegovy regularly reveal marionette lines that were padded by facial fat.
  • Genetics. A longer face and a stronger DAO tend to bring them on earlier.

What treatments do we offer for marionette lines in Toronto?

There is no single treatment, there is a stack, and it rarely starts with filling the line directly. The work combines injectable structure and muscle relaxation with non-injectable skin tightening.

Structural filler around the line, not into it

The most effective single move is restoring volume around the line rather than into it. Cheek filler lifts the descended tissue, pre-jowl and chin filler straighten the jaw contour and bring the area forward, and a moderate marionette often improves substantially before we touch the line at all. When the line itself needs direct work, we use a soft filler with a cannula in a small amount per side.

We usually start with one to three syringes across the relevant areas, then reassess at four weeks. Maintenance is typically every 12 to 18 months.

Botox to the DAO

A few units per side into the depressor anguli oris relaxes the downward pull on the mouth corner, so the corners lift, the line softens, and the resting face reads more neutral. This is one of the highest-value low-dose Botox treatments we do. It is also technique-sensitive, because placement that is too medial or too superficial can affect your smile, so this is a treatment to have done by an experienced injector.

  • Botox is $10 per unit, and the DAO is a small, low-unit treatment.
  • Sessions: every three to four months.

Sculptra stimulates your own collagen across the lower face over three to six months. We use it alongside HA filler for people who would rather build their own collagen than top up every year. The result builds gradually and lasts well over a year.

Injectable versus non-injectable for marionette lines

Filler, DAO Botox, Sculptra and Radiesse are injectable, and together they rebuild structure, relax the muscle pulling the corner down, and soften the line. Morpheus8 and PDO threads are non-injectable, and they tighten or reposition the skin. Marionette lines are a structural and muscular problem more than a surface one, which is why the injectable side usually does the heavy lifting and the skin work supports it.

What combination approach works?

For someone in their mid-40s to mid-50s with moderate marionette lines, a typical plan is:

  1. Cheek filler for structural lift from above.
  2. Chin and pre-jowl filler to straighten the jaw contour.
  3. DAO Botox in a small dose every three to four months.
  4. Direct marionette filler only if still needed at the four-week reassessment.
  5. A Morpheus8 course for skin quality.
  6. At home, a prescribed retinoid, daily mineral SPF, and attention to sleep position.

What results can I expect, and how soon?

  • Week 1: filler swelling settles and the volume is visible.
  • Week 2: Botox takes effect and the corners lift.
  • Week 4: final filler result, and the point we reassess.
  • Months 3 to 6: Sculptra and Morpheus8 compound.
  • Months 12 to 18: first maintenance round.

What you can expect: a clear reduction in how deep the line looks, a more neutral resting expression, a straighter jaw contour, and less of the angry-in-photos effect.

What you cannot expect: complete erasure of a deeply etched line of many years, permanent results without maintenance, or the face of a 25-year-old at 55.

Who is a good candidate, and who is not?

Most healthy adults with early to moderate marionette lines do well. Treatment is not the right move in these cases:

  • Pregnancy or breastfeeding.
  • Active infection in the area.
  • Blood thinning that cannot be paused.
  • Significant tissue descent that is honestly surgical, where we refer.
  • A known allergy to HA or lidocaine, or an active autoimmune flare for biostimulators.
  • Body dysmorphic concerns, which our MD screens for.

Common questions

Can I just get filler in the line?

You can, but it is usually not the best result on its own. Filling only the line, without restoring the support around it, is the main reason marionette filler can look heavy or unnatural. Treating the cheek, chin, and muscle first gives a softer, more natural outcome.

Why does marionette filler sometimes go wrong?

The usual reasons are too much product placed directly into the line, ignoring the structural causes around it, or poor technique near the mouth. Our approach is to build support first, use a small amount of soft filler only where needed, and reassess before adding more.

How does Botox help marionette lines?

A small dose into the DAO muscle relaxes the downward pull on the mouth corner, so the corner sits higher and the line softens. It pairs well with filler rather than replacing it.

How long do the results last?

HA filler is roughly 12 to 18 months, Sculptra well over a year, Radiesse around 9 to 12 months, and DAO Botox three to four months.

Does it hurt?

With numbing and a cannula, filler around the mouth is usually well tolerated, around 3 to 4 out of 10.

Will I bruise?

Some bruising is common around the mouth and usually settles within a couple of weeks. The pre-treatment instructions we send reduce it.

My marionette lines appeared after Ozempic. Can you help?

Yes, this is a common reason people come in. We prefer your weight to be stable first, then restore the lost structural support around the mouth and jaw.

Can men get marionette line treatment?

Yes. The approach is the same, with placement kept appropriate to a masculine lower face.

Should I choose filler or Sculptra?

Filler gives an immediate result, and Sculptra builds collagen gradually. Many plans use both, and we recommend based on your anatomy at the consult.

When is surgery the better option?

For heavy tissue descent and jowling, a lower face lift gives a result non-surgical treatment cannot match, and we will tell you honestly and refer.
